Order Yellow Dot Packets

Have you thought about what the immediate aftermath of a serious crash might be? Would you be conscious? Able to speak? Able to tell first responders what medications you’re taking, any special medical considerations and who to contact? These considerations and the importance of administering the most effective care as quickly as possible to crash victims led to the creation of the Yellow Dot Program. The Yellow Dot program is a statewide program providing first responders with the ability to more quickly respond to your needs in case of an accident. Each FREE red packet contains:

  1. A yellow sticker to place in your windshield on the drivers side of the car
  2. An information pamphlet explaining how Yellow Dot works
  3. A form to be filled out for each occupant that might travel in the car:
    1. At a minimum, include emergency contacts, allergies, special conditions, and any medications
    2. If there are no allergies, conditions, medications, etc, write 'None' or 'NA'
    3. Consider any information you would want first responders to know if you were in a crash and unable to respond
      1. This could include instructions on who to call if your pets are in the car with you, or that someone who is nonverbal is riding with you-think of anything you would want EMS to know so they can expedite care

Once the form is filled out, place it in the red packet. Put the packet in your glove box. Put the yellow sticker in your windshield. EMS Providers are taught that if they see the sticker in the windshield, they should look in the glove box. In an emergency, time is of the essence!

INDIVIDUALS: Request a packet for each car in your household (your friends and families, too). Have a family event where you all sit and fill them out together, put the sticker in your windshield, and packet in your glove box!

ORGANIZATIONS: We can send out multiple packets to you along with a poster explaining the program. Place them in your waiting room, entry way, or distribute to your families and clients.
